Job Description

This position is responsible for providing services to hospice clients, families, and caregivers, addressing a wide range of psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. The MSW develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Team.

Required Qualifications:

  • Master's Degree - In Social Work from an accredited School of Social Work.

  • Washington Advanced Social Worker License upon hire or,

  • Washington Advanced Social Worker Associate License upon hire or,

  • Washington Clinical Independent Social Worker License upon hire or,

  • Washington Clinical Independent Social Worker Associate License upon hire.

  • 1 year - Health care setting.

Preferred Qualifications:

  • 2 years - Health care setting.

  • Hospice care setting.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
